The sweepstakes is open to all current LeadingAge or Value First members. Members with multiple locations may enter once per location, but a Member may not win more than one Prize. Members who utilize Value First as the primary group purchasing organization for their food program at the time of entry will automatically be entered into the drawing. For Members who are not utilizing Value First as the primary group purchasing organization for their food program, please complete and send the form below with the following information: member contact name, email address, phone number, member organization name and address.

Overview of Official Rules
For Members who are not utilizing Value First as the primary group purchasing organization for their food program at the time of entry, we require the following information to enter: member contact name, email and your most recent full calendar month of food purchase history in excel format that includes the following data:
• Manufacturer item number
• Current manufacturer name
• Complete product description
• Quantity purchased of Designated Unit of Measure
• Price paid for each item on 1st day of the most recent full month
• Designated purchase unit of measure (bx, cs, ea, etc)
- Package detail for unit of measure Qty (eg. BX of 100)
